Neck pillow or regular pillow? What really helps with neck pain
Neck support pillow or classic pillow: the differences, what the Stiftung Warentest 2025 test shows, which pillow is right for whom – and why height is more important than shape.
Inhalt
In short: A foam neck support pillow can help, but it's not a cure-all: In the Stiftung Warentest test (issue 9/2025), only 4 out of 14 neck support pillows provided good support in every sleeping position. For both types of pillows, the key is that the height suits your sleeping position and shoulder width. A classic pillow with adjustable height is a good alternative if you change positions at night or prefer something soft. If the pain persists, have it checked by a doctor.
If you wake up in the morning with a stiff neck, you'll quickly find yourself wondering: Do I need a special neck pillow – or is a good regular pillow enough? Here you'll find the differences, what independent tests show, and how to tell which pillow is right for you.
What is the difference?
| Neck support pillow | Classic pillow | |
|---|---|---|
| Material | mostly foam or memory foam, sometimes latex | Down, feathers or fiber like Primaloft Bio |
| Shape | firm, often with a wave or hollow | moldable, adapts |
| Height | fixed, sometimes adjustable via inserts | via the filling, adjustable in some pillows |
| Sleeping feel | firm and supportive | soft and cozy |
| Changing positions | more difficult depending on the shape | flexible |
| Care | usually only the cover is washable | often the entire pillow is washable |
What does Stiftung Warentest say?
In issue 9/2025, Stiftung Warentest tested 14 neck support pillows between 40 and 164 euros, most made of foam. The results ranged between "good" and "satisfactory". The most important findings:
- Only 4 out of 14 pillows provided good neck support in every sleeping position.
- Many pillows were difficult or impossible to adjust, and instructions were often missing.
- The testers were often only able to weakly prove the advertised memory effect in the laboratory.
- Every pillow helped at least one test person to have a relaxed position – but many were not suitable for everyone.
The support properties accounted for 40% of the assessment, and sleeping comfort for 20%. Classic pillows made of down or feathers were not part of the test. Which exact models were tested can be found at Stiftung Warentest.
Which pillow is right for whom?
A neck support pillow may be right for you if you …
- mostly sleep in one position, such as on your back or side,
- like a firm sleeping feel,
- it was recommended to you by a doctor or physiotherapist.
A classic pillow is more likely to be right for you if you …
- often change positions at night,
- like a soft, cozy pillow,
- sleep on your stomach and need it to be very flat,
- want to wash the entire pillow regularly.
Height is more important than shape
Regardless of the pillow type: the head and spine should form a straight line. Side sleepers therefore need more height, back sleepers a medium height, and stomach sleepers a very flat one. How to find your height is explained in the guide Which pillow for neck pain?
Classic, but adjustable
Traumwelt does not offer foam neck support pillows. If you want a soft pillow but still want to determine the height yourself, there are two options:
- 3-Chamber Hybrid Pillow: Down outer chambers and an inner chamber made of Primaloft Bio, which you can remove or add via a zipper.
- Primaloft Bio Pillow: vegan, removable filling – and if you need more height, you will receive additional filling material free of charge.
The 3-Chamber Down Pillow is flat, soft, and not adjustable – it is intended for stomach sleepers, not as a support pillow for the neck.
Set the height yourself3-Chamber Hybrid Pillow – Down & Primaloft BioSoft as a down pillow, with adjustable height via the removable Primaloft Bio inner chamber.View pillow
Frequently Asked Questions
Is a neck pillow healthier than a regular pillow?
Not necessarily. The key is whether the height and shape suit your sleeping position. In the Stiftung Warentest test, only 4 out of 14 neck support pillows provided good support in every position.
High or low side – how do I lie correctly on a neck pillow?
This depends on the model and is stated in the instructions. As a rule of thumb, side sleepers usually use the higher side, while back sleepers use the lower side.
How long does it take to get used to it?
Often a few nights. Therefore, change the height in small steps and give yourself some time after each adjustment.
Can I sleep without a pillow at all?
For stomach sleepers, a very flat or even no pillow is sometimes enough. Side and back sleepers, on the other hand, need height so that the neck does not bend.
When should I see a doctor for neck pain?
If the pain lasts for weeks, radiates into the arms or hands, or is accompanied by numbness or tingling. A pillow does not replace treatment.
